High Acuity Social Worker

At Fenway Health, we believe health equity starts with affirming care. We are currently hiring a High Acuity Social Worker to join our mission-driven team supporting LGBTQ+ individuals, people living with HIV, and marginalized communities through behavioral health and community outreach.

What You’ll Do:

• Provide 1:1 therapy using harm reduction and trauma-informed approaches

• Support and supervise CHWs who provide community-based case management

• Conduct clinical assessments and public health intake screenings

• Respond to drop-in clients with emotional support and crisis intervention

• Collaborate with public health teams and medical providers to ensure continuity of care

What You Bring:

• MSW + LCSW (MA)

• Experience working with LGBTQIA+ individuals, communities of color, and underserved populations

• Dedication to health justice and accessible care

• Familiarity with HIV care, substance use treatment, and social determinants of health
